How The Kind Mind Collective handles personal, health, disability and funding information shared through an enquiry or referral.

Who collects the information

The Kind Mind Collective collects information submitted through its referral form. CordoCare provides the secure software used to receive and manage that information on behalf of the practice.

What may be collected

Depending on the enquiry, this may include names, contact details, date of birth, referral source, health and disability information, support needs, accessibility preferences, funding information, Medicare or NDIS referral details and documents you choose to upload.

Why it is collected

The information is used to assess and respond to the enquiry, consider whether the service is a suitable fit, communicate with the person or authorised referrer, prepare onboarding and, if services begin, meet clinical record-keeping, funding and billing obligations.

What is required

The form marks the minimum fields needed to respond and safely assess the enquiry. Other details and document uploads are optional. If required contact, funding or referral information is not provided, the practice may be unable to assess the enquiry or bill the requested service.

Information from a referrer

If another person makes the referral, they should have the person's authority or another lawful basis to share the information. The practice may contact the person directly to confirm details and consent before treatment starts.

Who may receive it

Information is limited to authorised people working for The Kind Mind Collective and service providers needed to securely operate the practice. It may also be shared with a funder, referrer or another health professional where the person has authorised this, where it is needed to provide the service, or where disclosure is permitted or required by law.

Consent and treatment

Sending a referral consents to the collection and use described in this notice. It is not consent to treatment and does not guarantee that a service or appointment will be offered.

Access, correction and privacy concerns

You can ask to access or correct personal information, withdraw an enquiry before services begin, or raise a privacy concern by emailing thekindmind@pm.me. Requests will be handled in line with the Australian Privacy Principles, the Privacy Act 1988 (Cth) and other applicable health information requirements.
