How The Kind Mind Collective handles personal, health, disability and funding information shared through an enquiry or referral.

The Kind Mind Collective collects information submitted through its referral form. CordoCare provides the secure software used to receive and manage that information on behalf of the practice.
Depending on the enquiry, this may include names, contact details, date of birth, referral source, health and disability information, support needs, accessibility preferences, funding information, Medicare or NDIS referral details and documents you choose to upload.
The information is used to assess and respond to the enquiry, consider whether the service is a suitable fit, communicate with the person or authorised referrer, prepare onboarding and, if services begin, meet clinical record-keeping, funding and billing obligations.
The form marks the minimum fields needed to respond and safely assess the enquiry. Other details and document uploads are optional. If required contact, funding or referral information is not provided, the practice may be unable to assess the enquiry or bill the requested service.
If another person makes the referral, they should have the person's authority or another lawful basis to share the information. The practice may contact the person directly to confirm details and consent before treatment starts.
Information is limited to authorised people working for The Kind Mind Collective and service providers needed to securely operate the practice. It may also be shared with a funder, referrer or another health professional where the person has authorised this, where it is needed to provide the service, or where disclosure is permitted or required by law.
On eligible public pages, the site uses Google Analytics automatically to understand which broad parts of the website help people find the practice and move towards an enquiry. There is no analytics choice popup.
The site sends a coarse virtual page category such as practice overview, service information, resources, funding information, privacy, or contact information. It does not send the condition-specific page path or page title. Page referrers, query strings and URL fragments are suppressed. The site may count a small allowlisted set of public-site actions such as selecting a broad information pathway or choosing the practice email link.
The analytics code does not read or send referral-form answers, referral-form interactions, successful-submission events or uploaded documents. It does not intentionally send names, email addresses, phone numbers, dates of birth, Medicare or NDIS details, funding choices, free-text responses, search text or user IDs. Advertising storage, Google Signals and ad personalisation remain disabled. Analytics cookies expire after no more than 90 days and are not extended on each visit.
Google Analytics storage is set to denied for visitors in the European Economic Area, the United Kingdom and Switzerland. Those regional settings are based on the visitor's location, not the practice's Australian location.
As with other internet analytics services, Google necessarily receives standard connection and browser information, which can include an IP address, device information and analytics cookie identifiers. The secure referral form is provided by CordoCare and is kept separate from Google Analytics. The Google tag does not load on the secure-form page. Referral submissions and outcomes remain in the practice's operational system and are reviewed only in aggregate for business reporting.
You can prevent Google Analytics from running by blocking analytics cookies or scripts in your browser, or by using the Google Analytics opt-out browser add-on. Blocking analytics does not affect access to the website or care.
Sending a referral consents to the collection and use described in this notice. It is not consent to treatment and does not guarantee that a service or appointment will be offered.
You can ask to access or correct personal information, withdraw an enquiry before services begin, or raise a privacy concern by emailing hello@thekindmindco.com. Requests will be handled in line with the Australian Privacy Principles, the Privacy Act 1988 (Cth) and other applicable health information requirements.
